This is an older image of mine, which I revisted the post-work on now that I've got a bit more experience with monochrome conversion.
The model is "Mrs T." - a minor media figure, who wanted a valentines' day photo to send her husband as a surprise. Our arrangement was that I'd only shoot photos that did not show enough to reveal her identity to anyone but her husband (who, presumably, could recognize her).
This was lit with a pair of softboxes (look at the shadows cast) angled slightly together.
This image is NOT STOCK. See [link] for details. Idiots who grab this and photoshop on it will not even get a "you idiot..." note, just a violation report and a removed deviation.
